Idea Lady gave a birth 4 days ago, now she feel low, no previous personal or family history, she don't want to take care of the newborn baby.

Idea after 5 days from delivery, the lady suffer from sever anxiety and she hear orders and respond to them, but have no thoughts of harming the baby or herself.

most appropriate management option :
-reassurance
-interpersonal therapy
- SSRIs
-antipsychotic and to psychiatric unit
-assessment by psychiatrist.

2 for no 2 it is REASSURENCE AS SHE IS HAVING ANXIETY WHICH COULD BE BECUSE OF NEW RESPONSIBILITY OF BABY and there is no family or personal history of psychatric disorder

I will agree about the first, but i think the second case is pshychosis, so I'm rounding between the last 2 items.

This fits into the category of postpartum depression.
The key point is :she does not want to take care of the newborn baby .
The answer/option would be interpersonal therapy. If that does not help then the second option is SSRI
In postpartum blues symptoms do not interfere with a mother's ability to function and to care for her child

The first case could me [ Maternal blue], reassurance can be attempted as there is no other risk factor.

The second agree with cpeedasha as the symptoms start acute after Day 5.

Read what the others have said .... you cannot reassure a woman who 'does not want to look after her own baby'!
