After abd Hysterectomy

when can patient resume sexual intercourse
is it 6 weeks?

when can she go back to work
depends on what she does, no heavy lifting for 3 mths

am i right? please advice

Six weeks is the general rule for everything if, and I stress the word if, they have recovered uneventfully. If there are problems then each case needs individual assessment.

There is little scientific rationale for 6 weeks and the time may relate more to post-natal follow up than gynae.

Patients can do things before six weeks (lifting, work, driving, and yes even sex if they feel capable. The more strenuous activities (I'll leave you to decide) require the go ahead from the physio who is best placed to assess capability. I always tell patients to see how they feel and to take things easy when the resume whatever it is they want and to stop if they have discomfort - seems a sensible thing to do. As far as driving is concerned get them to check with their insurers as each company has a different policy and without their permission you would not have a leg to stand on if you had an accident. It's all about the ability to perform an emergency stop.

When I worked in east London, I saw lots of patients with vaginal lacerations (lots is a bit of an exaaggeration, two actually) they both denied any activity with non-human partner or non live apparatus Laughing, Julian will probably remember one of them.

I think one of them had a hysterectomy many months before and the other was quite young with her uterus intact! I suppose that area had a population with lots of men with extraordinary physiques Shocked

This experience always comes to my mind whenever advising patients about resuming intercourse.

Joking apart I think one good way is to get the patient to feel their vagina as specially in case of vag hyst and repair, things may have changed dramatically in shpe and size (specially if done by some of my colleagues) and leave the judgement to the patients common sense.

This can also be a preventive measure for psychosexual problems. I remember watching a video (a legitimate one Wink ) for my DFFP (now DFRSH) psychosexual training where a lady came to change her HRT as believed she had lost interest in sex completely since her vag hyst. further Miss Marple style questioning revealed that she thinks her vagina is "a mess". Miss Marple got her to examine herself and life became beautiful again.
