Please can I ask [ What is incident Form ] ?? Embarassed
I know it is part of Risk management but how it looks like ??
I ask that because I am not UK based and we not really have this protocol in our hospitals... Confused

When an Adverse Incident occurs, the law says that you have to report it to your employer as soon as possible.

All incidents (including accidents and near misses) which affect the service, premises and property owned and occupied by the hospital and which may involve patients, staff and visitors, or other people who come into contact with the Hospital/Trust's activities.

Any member of staff who experiences, discovers, witnesses or is notified of an incident (if not already reported) can complete the form.


Information relevant to the incident is summarised and trying to avoid giving opinions.
Details of any injury or damage sustained and include what immediate action was taken is included.
If the person who has had the adverse incident is a patient it maybe necessary to take their consent to their details being submitted on to the form.
The information in the incident form is used to help in the investigation of why the incident occurred so as to try and prevent it happening again.
The information is secure and only accessible by a few persons who have access rights and would only be used when absolutely necessary as far as possible anonymously.

Adverse incidents can include a wide range of topics , most importantly patient safety and also anything from missing case notes, inebriated colleagues, rude/abusive colleagues, rude staff, rude patients, certain operative complications, medication errors, prescribing errors to theft in the hospital.

Each hospital/trust/department may have its own policy regarding reporting incidents. Some have electronic databases and some have paper databases. Forms are made available easily in a designated place or the hospital/trust website provides a link to the forms.

Incident forms are an important part of risk management process. The goal is to identify incidents and risks before they happen and, where possible, avoid them. It is also an effort to ensure risks are appropriately identified, assessed, controlled and where possible eliminated. It is also an important tol to enable preventative measures to try to reduce the number of incidents and risks experienced and to minimise the possible harm when they do occur.

A no-blame policy, open and honest culture is encouraged in adverse event reporting. Periodic reviews are undertaken and changes implemented as necessary.
