Is it the policy in UK not to remove the fibroid, how much ever large it is, if asymptomatic ?

That is not true. I don't think fibroids are managed much differently in the UK.

In general, with few exceptions, we have guidelines as opposed to policies, leaving room for individual practitioners to apply their clinical judgment. Examples of these are Green top guidelines in RCOG website and the NICE guidelines.

About large asymptomatic fibroids, the decision would be made in conjunction with the patient (like every other treatment decision) considering many factors, including her age, parity, her wishes to concieve, the size of the fibroid etc. There is not prescriptive way of managing these and as usual we have conservative, medical, surgical and other treatment modalities.

Surgery is certainly AN option and can be in the form of myomectomy(laparoscopic or open) or hysterectomy (total or subtotal, laparoscopic or open) but obviously should be considered after considering other options.

The "other" treatment modalities canso be considered and they include Uterine artery embolisation and MRI-guided focused ultrasound.

Again as a general rule, everything can be an option in treatment of everything (well, there are exceptions Wink )
Is permanent sterilisation an option when discussing contraception with a 19 year old? Yes it is and it depends on many factors.
