A patient came in labour with history of epilepsy but fit free for 1 year. Blood pressure is raised at the time of admission with 2 + protienuria. she delivers normally and 10 min after delivery gets a fit and another one after 10 min. All this happens at 5 am. Regsitrar is doing instrumental delivey in next room. Midwife (who is the lead midwife of delivery suite) thought it was an epileptic fit and did not inform registrar as patient has regained consciousness and encourages patient to breast feed!!!
Registrar subsequently asked about any other problems in delivery suite and was told every thing is fine!!!
After handover around 10 am patient fitted again developed PPH, was rushed to theatre and then to ITU.
How do you write risk management report? |
