50% of post menopausal bleeding with pyometra have endometrial carcinoma - Churchill's pocketbook of Obs & Gynae by Brian Magowan
Does this mean that Endomtrial carciinoma's characteristic features include pyometra.
Nick any takers on this? |

No, it does not mean that pyometra is a characteristic feature of Endometrial cancer. 50% of post menopausal bleeding with pyometra have endometrial carcinoma.
When we say 50% of post menopausal bleeding with pyometra have endometrial carcinoma- it only means when we encounter the combination of postmenopausal bleeding + pyometra---50% of them have endometrial cancer. (Individually pyometra is not characteristic of endometrial c ancer.)
It is worthwhile to remember that only 20% of patients with post-menopausal bleeding will have a malignant origin for their bleeding, of which over 50% will be due to endometrial carcinoma. Which inturn means only 10% of postmenopausal bleeding have endometrial cancer.
However when in combination with PMB then it becomes significant.
There is one study reported in "Maturitas", Volume 55, Issue 4, Pages 334-337. This reported There are scarce reports about the association of PMB and intra-uterine fluid accumulation in the English literature. Carlson et al. found eight patients with intrauterine fluid accumulation and PMB. Four patients had various types of gynecological malignancy: one in the ovary, one in the fallopian tube, one in the cervix and one had endometrial cancer.
Various authors have reported an incidence of 4–14% of intra-uterine fluid collection as an incidental finding by ultrasound in asymptomatic postmenopausal women.
Over 90% of endometrial carcinomas present with irregular or post-menopausal vaginal bleeding. -- and that is indeed a characteristic feature of endometrail cancer. (On the other hand only around 10% of postmenopausal bleeding is due to Endometrial cancer)
Hope this helps!
Again, sorrrrrrrrry...I am not Nick
By the way-- I am a great fan of this book and Brian Magowan-- A great teacher . People in Borders --really lucky to have him. He wrote this book whilst preparing for his membership exams--ofcourse the book has seen updated editions since then. |
