Fistulae between the vagina and the gastrointestinal tract (enterovaginal, colovaginal, rectovaginal)- uncommon.
MCC of colovaginal fistula -diverticular disease. Of the minority of patients with diverticular disease who form fistulae, 25% will be colovaginal.
Other causes of colovaginal fistulae- irradiation, pelvic surgery, malignancy, abdominal hysterectomy, abscess formation, foreign-body perforation, inflammatory bowel disease (overwhelmingly Crohn disease), congenital malformation, and trauma.
Most women with colovaginal fistulae would have undergone previous hysterectomies.
Classic symptoms -passing flatus or feces PV, brownish and/or malodorous vaginal discharge, vaginitis, fecal incontinence, or (less commonly) constipation. Rarely asymptomatic. |
