You are asked to conduct an audit of confidentiality by your clinical Director. During this some trainees report the following practices, all of which concern them.

1.Pathology reports are sent by fax to patient reception area of clinic.
2.Verbal reports are given to Consultant on telephone.
3.Results of cervical biopsies are sent to a neighbouring trust's cytopathology department because they reported the cervical smear.
4.Malignant diagnoses are submitted to Regional Cancer Registry.
5.Patient histories are discussed at multidisciplinary team meetings.

Which one/s breach/es Caldicott guidelines?

A)Cancer registry staff who are not responsible for patient care would recieve the information.
B)Patient's details will be discused with staff not directly responsible for patient's file.
C)Reception staff not directly involved with patient care would see the reports.
D)Reports maybe lost by postal service
E)Verbal reports maybe transcribed incorrectly.

Nick, My information regarding conducting an audit is NILL Embarassed Crying or Very sad
although I think it is an OSCES subject.
Cpeed, plz, can you give the reference of your statements Question

Emak,
Caldicott guidelines are guidelines for releasing information about patients, case notes, and x ray films. They provide protocols for essential sharing of information. This MCQ is regarding those guidelines.

They are very important but much more likely to come up in the OSCE.

I think the MCQ formet is good but I'll move this to the "How Things Work in the UK" Forum to ensure this important topic does not disappear in a quagmire of Past Paper questions.
