Station 2 [with examiner]
U r doing a hysterectomy with SHO, found that bladder is opened, what will be ur immediate actions?
What u will do to complete procedure?
What will be ur postop instructions?
What u will tell to the patient? |

U r doing a hysterectomy with SHO, found that bladder is opened, what will be ur immediate actions?
. see the extend of injury
. Foley's should be in place preoperatively
.
What u will do to complete procedure?
. if the damage is small, we can suture the bladder by 2 layers, vicryl 2/0
. Urological surgeon should be summoned if the defect is large or difficult
. test bladder after finishing the suturing[ water tight test]
What will be ur postop instructions?
. Catheter should remain in place for 10 days to encourage good healing.
What u will tell to the patient?
. we tell her that there was a damage to the bladder, occur incidentally, [ if there is adhesion I should mention], and inform her that this complication can occur infrequently [0.4-0.6 %] of cases. Repair is done by assistance of urologist and it carries good prognosis usually.

You forgot one the most important words of the OSCE!
"SORRY"
This is not an admission of 'guilt' or 'fault' but that you appreciate the importance of the situation.
Then offer her / guarantee her follow-up and, if requested, support to make a complaint.
The rest is good. |

Every hospital/NHS trust will have a local complaints procedure/system in place. Inform the patient that the aim is to provide a friendly and reliable service to ensure that the patients get the treatment they need; and when sometimes things go wrong; there are procedures in place to help them to voice their concerns. Provide them with contact details of the Complaints Manager in the hospital.
Inform them also about the Patient Advice and Liaison Service (PALS) in the NHS Trust/ primary care trust (PCT) which is a confidential service that is designed to help patients get the most out of the NHS.
If they require help in making their written complaint then an independent organisation called ICAS (Independent Complaints Advocacy Service) is available to support them in pursuing their complaint about the NHS/hospital. (Provide them with contact addresses and numbers) |
